1 Sun Jianqin 2 Yuan Wuke 3Huang Jiani
Sarcopenia is a common disease in the elderly population that is associated with increased adverse outcomes including falls functional decline frailty mortality severely affecting quality of life and increasing health care costs. There are crucial steps to delay the development of sarcopenia and maintain the muscle health of the elderly like early screening early diagnosis and early intervention. Nutrition plays a key role in the treatment of sarcopenia. However nutritional interventions have not received sufficient attention in practice. The need for sarcopenia prevention and therapy is becoming more urgent because of the COVID-19 pandemic and the aging of the population. To highlight the important role of nutrition in the prevention and treatment of sarcopenia the Asian Working Group for Sarcopenia AWGS established a Special Interest Group SIG to form 14 nutritional consensuses from 6 themes for the elderly ≥60 years of age based on evidence-based medicine methods to retrieve papers published in 2016-2021 related to muscle health in Asians 1. nutritional status malnutrition and screening 2. diets and dietary patterns 3. nutritional supplementation 4. lifestyle interventions 5. outcomes and assessment 6. impacts of COVID-19. The consensus emphasizes the impact and role of nutrition and the COVID-19 pandemic on muscle health and sarcopenia in older adults providing a practical framework for addressing muscle health in the community. The article interprets the core information of the consensus.

555 Potential preventive effect of time-restricted eating on cancer
1Fang Ruoxin,2Yan Ling,1Liao Zhengkai
Intermittent fasting is a novel dietary intervention strategy that has been demonstrated to improve the efficacy of malignancy treatment and reduce complications. Time-restricted eating is a specific form of intermittent fasting. It usually restricts only the daily eating window and not calories. In addition to weight management and improving metabolism like other intermittent fasting methods time-restricted eating also synchronizes circadian rhythms and affects autophagy levels which has potential anti-aging effects. Some studies have shown its potential to prevent tumorigenesis and slow tumor progressions. Moreover due to the unsophisticated implementation and mild side effects time-restricted eating is safe and well followed. It may become an adjuvant therapy and even a healthy lifestyle suitable for the long-term execution of cancer patients. This paper expounds on the potential impact mechanism of time-restricted eating on the prevention and treatment of cancer from three aspects improving obesity and metabolic disorders synchronizing circadian rhythm and up-regulating autophagy and current research progress related to tumors. The purpose of this review is to provide a theoretical basis and direction for future research to explore the potential preventive effect of time - restricted eating as a nutritional intervention on the occurrence of cancer and the role of treatment or adjuvant therapy for cancer. It is conducive to improve the nutritional strategy for cancer patients and improving the quality of life and prognosis of patients.

566 Clinical significance of nutritional intervention in four-level prevention strategy of Alzheimer's disease
1Xu Jun,1Jiang Jiwei,2Shi Hanping
With the rising aged tendency of population Alzheimer􀆳s disease AD the leading cause of dementia in the elderly is the greatest and sharp increasing challenge for global public health and social care in the 21st century. Recently 􀆵ATN research framework has emphasized the core role of amyloid β Aβ plaques and hyperphosphorylated Tau-protein in the AD continuum. However over the past two decades clinical trials of single-agent drug targeted Aβ and Tau have frequently failed to cure or prevent disease progression in AD. Ad is still based on symptomatic treatment and the representative drugs are cholinesterase inhibitors and memantine. AD is among the least well-served therapeutic areas for drug treatments. These evidence have suggested that AD as a complex and multifaceted disease its management should turn the attention from single-agent drug to diet and lifestyle regulation and risk factors control. However there are significant gaps in knowledge in the impact of modern dietary structure and lifestyle changes. The important value of brain health and nutrition intervention in AD still remain unclear. The multidisciplinary comprehensive management strategy of Neurology Geriatrics and Clinical Nutrition is yet to be established. This review summarized the current evidence on diet and nutrition in AD patients. We wish to propose the term of 􀆵four- level prevention strategy of AD and the effect of nutrition in each level of prevention to provide new direction for the comprehensive management of AD in China.

1Zhu Jianyun,2Xie Chunfeng,2Zhong Caiyun
Objective To explore the inhibitory effects of quercetin on the activity of A549 lung cancer stem cells. The role of quercetin on Wnt / β-catenin and Sonic Hedgehog SHH pathway was further illustrated. Method The lung cancer cell line A549 was cultured in vitro and treated with various concentrations of quercetin 0 5 10 25 50 100 and 200 μmol / L for 24 48 and 72 h cell vitality was detected by MTT assay. Lung cancer stem cells were isolated and enriched using sphere-forming assay. The effects of quercetin on the sphere formation and the expression of lung cancer stem cell specific markers were evaluated. The role of quercetin on the proliferation and apoptosis of sphere-forming cells was further determined by flow cytometry and western blot analysis. Finally the protein levels of key molecules of Wnt / β-catenin and SHH pathways were detected by western blot assay. Result Our results showed that quercetin significantly inhibited A549 cell viability in a time- dependent and concertation-dependent manner. Compared to the vehicle control group quercetin markedly suppressed the sphere formation ability of A549 cells and reduced the expression of lung cancer stem cell specific markers CD133 CD44 and aldehyde dehydrogenas 1A1 ALDH1A1 . Furthermore quercetin reduced proliferation and induced apoptosis of lung cancer stem cells. The levels of proliferating cell nuclear antigen PCNA Cyclin D1 and B-cell lymphoma- 2 Bcl2 were markedly downregulated while the expression of Bcl2-associated X Bax and cleaved cysteinyl aspartate specific proteinase Cleaved Caspase 3 was significantly upregulated upon quercetin treatment. The Wnt / β-catenin and SHH-related proteins phospho-glucogen synthase kinase-3β p-GSK3β Ser9 β-catenin c-Myc glioma-associated oncogene homolog 1 GLi1 GLi2 and SHH in lung cancer stem cells were also inhibited by quercetin. Conclusion Quercetin significantly suppressed the activity of lung cancer stem cells which might be associated with the downregulation of Wnt / β-catenin and SHH pathways.

665 Clinical value of control nutritional status for the prognostic evaluation of cervical cancer patients with FIGO ⅠB~ ⅡA stage
Jiang Li,Qian Hua,Yuan Donglan, Zhao Yinling, Bu Xinhua
Objective To explore the clinical value of controlling nutritional status CONUT in the prognostic evaluation of cervical cancer patients. Method A total of 122 FIGO ⅠB~ⅡA stage patients who received surgical resection for cervical cancer in Taizhou People's Hospital from January 2016 to December 2018 were included in this study. All patients were divided into CONUT <3 points and CONUT ≥3 according to the CONUT score. The distributions of baseline characteristics between the two groups were compared by χ 2 test or Fisher's exact test. The survival curves were plotted and the 5-year overall survival OS rates of patients in CONUT <3 points and CONUT ≥3 points group were calculated using Kaplan-Meier curve. Univariate and multivariate Cox regression analysis was conducted to identify the independent prognostic factors for cervical cancer patients. Result The frequency of pelvic lymph node metastasis 42. 9% vs 23. 7% χ 2 = 4. 758 P= 0. 029 and the proportion of FIGO ⅡA stage 64. 3% vs 42. 5% χ 2 = 5. 229 P= 0. 022 were significantly higher in the CONUT≥3 points group than in the CONUT <3 points group. Kaplan-Meier curve showed that the 5- year OS rates of patients in the CONUT≥3 points and CONUT <3 points group were 63. 4% and 86. 6% respectively χ 2 = 11. 409 P= 0. 001 . Subgroup analysis further confirmed that CONUT≥3 points was a significant predictor of poor OS for cervical cancer patients irrespective of FIGO stage FIGO ⅠB stage 65. 8% vs 95. 5% χ 2 = 7. 902 P= 0. 005 FIGO ⅡA stage 62. 7% vs 76. 1% χ2 = 4. 206 P= 0. 043 . In addition the univariate and multivariate Cox analysis revealed that preoperative CONUT HR = 3. 542 95%CI = 1. 529 - 8. 207 P = 0. 008 tumor size HR = 2. 668 95% CI = 1. 158 - 6. 149 P = 0. 021 and pelvic lymph node metastasis HR= 3. 542 95%CI = 1. 529-8. 207 P = 0. 003 were independent prognostic factors for cervical cancer patients. Conclusion CONUT was an effective tool of prognostic evaluation and it might have an important clinical value for cervical cancer patients with FIGO ⅠB~ⅡA stage.

672 Progression in nutritional management of cancer patients during chemotherapy
Liu Yining,Cui Jiuwei
Malignancy has become the focus of studies around the world. Patients with cancer are prone to be malnourished and cancer-related malnutrition results from multiple factors. In addition to the malignancies itself the direct cause of death in 20% of cancer patients is nutritional deficiency and progressive cachexia. Malnutrition reduces the immunity and anti-infection ability of the body leads to the decrease of tolerance and sensitivity during chemotherapy and seriously affects the quality of life and prognosis. The incidence of nutritional risk in cancer patients is 40% - 70% . Early screening of nutritional risks in patients with cancer during chemotherapy and giving adequate and reasonable nutritional support to change their malnutrition is a major problem to be solved at present. Numbers of studies have shown that nutritional therapy during chemotherapy can significantly improve the malnutrition status of patients and have a positive impact on the prognosis. A comprehensive treatment mode based on five-step nutrition therapy has been gradually developed. Some new supplements and drugs for improving nutritional status have shown unique efficacy in clinical studies and animal trials. Large - scale prospective clinical trials are aimed at exploring effective means to improve nutritional status and prognosis. Nutritional therapy runs through antitumor therapy We discussed the nutritional status of cancer patients as well as the causes during chemotherapy and nutritional support therapy in perichemotherapy was reviewed.

681 A review of amino acid metabolism in triple negative breast cancer
Liu Xinran, Liu Peng
Triple-negative breast cancer is a subtype of breast cancer that lacks the expression of estrogen receptor progesterone receptor and human epidermal growth factor receptor 2. Its clinical characteristics are high degree of deterioration easy recurrence high metastasis rate and low survival rate. This kind of breast cancer accounts for 15% - 20% of all breast cancer cases and the treatment lacks effective targets. Metabolic abnormality is one of the important characteristics of tumor diseases. Therefore analyzing the characteristics of triple-negative breast cancer from metabolic pathway is a new direction of personalized nutritional therapy. Recent studies have found that the metabolism of nutrients especially amino acids in triple-negative breast cancer cells is different from normal cells and is also different from other subtypes of breast cancer cells. This paper reviews the characteristics of amino acid metabolism and related regulatory mechanisms in triple-negative breast cancer cells. It is found that the levels of glutamate / glutamine cystine / cysteine methionine glycine serine and tryptophan are different from normal cells or other types of tumor cells and the transformation and transport of amino acids are also in an abnormal regulatory state. Based on this characteristic regulating the corresponding metabolic pathway by limiting the target amino acids may have a good effect on the treatment of triple-negative breast cancer. At present nutrition deprivation has become an adjuvant treatment strategy for cancer cells. The in - depth study of the metabolic characteristics of the disease is of great significance for finding the target of nutrition therapy.
